CONSULTATIONS & ASSESSMENTS

A consultation is usually the first step. We make an assessment based on the whole picture – health, functional abilities, family and social support, physical environment, and financial situation. We then create a Care Plan which identifies the problems and outlines various options which can help solve those problems. We also provide referrals to other professionals and service providers when appropriate. Often this is enough for the client or client’s family to handle the situation from there. If additional services are needed, we explain how we can provide such services and give a clear quote of our fees.

SHORT-TERM TASKS

Sometimes a person or family needs help with a specific task, such as locating and applying to a good nursing home, arranging for home care service from a reliable agency, applying for Medicaid to pay for long-term care, etc. Our expertise and knowledge of services in the area can help with these tasks.
